Guest Vendors
Businesses that are registered with another municipality and are not located in Park Forest are considered a Special Events Outside Vendor. Any businesses handling, preparing, or selling food must comply with all applicable food safety and health regulations. All businesses, whether it is a Corporation, LLC, or Assumed Business Name (DBA) must be in "good standing" and properly registered in accordance with Illinois state law.
Food Vendor

- In the "description of operations" box, please provide:

Location of Event(s) - The Village must be listed as an additional insured. Therefore, the "certificate holder" must say:

- You are required to provide a ANSI Accredited Food Protection Manager Certificate
- You must also provide a Illinois Retail Sales Tax Certificate that can be obtained by clicking here

- Please note that there is a flat fee of $55 for each event that covers the health inspection

How to Submit Applications
- Applications must be submitted to Arika Rogers, Business Licensing, at arogers@vopf.com
- INCOMPLETE APPLICATIONS WILL NOT BE ACCEPTED
Important Notes
- First-come, first-served basis
- Please allow up to five business days for processing if your application is complete
- Food vendors will be inspected by the Cook County Department of Public Health at the beginning of each event
- Your event certificate will be issued on-site once you pass the inspection
- Peddlers will receive their certificates from the event coordinator on the day of the event
